§ 12-132. Issuance of license.

  • Application for a billiard room license within the corporate limits of the City of Cornelia shall be made to the city clerk upon a form to be supplied by the city clerk for this purpose. The license application shall include the following information:

    (1)

    Name, address, age and telephone number of the person providing information for the application on behalf of the business; the name, address and telephone number of the business; the name, address and telephone number of the owner(s) of the business if the business is not a publicly held corporation; if a corporation or partnership, the name and address of its' registered agent; and date of the application.

    (2)

    Location of the billiard room.

    (3)

    Name, address and telephone number of the owner of any bona fide coin-operated amusement machines located within the billiard room and a copy of the owner's master license.

    (4)

    Name, address and telephone number of any other business owned or operated by the licensee within the corporate limits of the city.

    (5)

    List of any other licenses or permits from the City of Cornelia held by the registrant.

    (6)

    Any violations of any federal, state, county or city laws or ordinances relating to gambling, the operation of a billiard or pool room, or the operation of bona fide coin-operated amusement machines (as defined in O.C.G.A. § 48-16) by the registrant. Such previous violations may be considered by the city to determine whether or not it is in the best interests of the public safety, health and welfare of the city to issue a license under this division.

    (7)

    Every registrant operating a billiard room shall provide to the city clerk the address and a description of the building in which such business will be located, and a diagram of the interior showing all entrances and the location of all coin-operated amusement devices and machines, billiard tables and pool tables, with the serial numbers of said coin-operated devices and machines and tables noted thereon.

    (8)

    Upon providing the information required by this section, a review by the city clerk of the information provided in the application, and consideration of the application and approval of the license by the city commission, a license shall be issued to the business.

    (9)

    Upon issuing a license, the city shall provide the licensee with a copy of the ordinance from which this section derives.

    (10)

    A license issued in accordance with this division shall be valid from the date of its issuance until December 31 of the year in which the license was issued.

    (11)

    Application for such license shall be made to the city on an annual basis.
